Fielding Graduate University considers the student's legal name as the one supplied by the student on their signed enrollment application.  The Registrar's Office requires two items of documentation to process a legal name change. The following forms of identification will be accepted as documentation for name changes: marriage license, divorce decree, driver's license, state ID card, military ID card, passport, or court order.

A copy of the new social security card is required for changes in social security number.

Students must request name and social security number changes in writing, with documentation enclosed.
